To submit a public information request for official documents, body camera footage, or other public records, please complete the form provided below and submit it to our office at 505 Railroad Avenue, Centreville, MD, 21617, or email it to Officer Coordinator, Dawn Becker at dbecker@qac.org.

A fee of $10, payable by check or money order, is required for each copy of a report requested. For requests involving camera system evidence videos, a fee of $75 per incident per device will be charged for videos with a total run time of less than one hour. Additional fees may apply.
